This Product Description Addendum (the “Product Addendum”) will become part of the executed agreement between Aclaimant, Inc. and Customer that references this document. The Product Addendum describes the functionality of the Services identified on an Order Form entered into between Aclaimant and Customer.

This Product Addendum was updated December 14, 2021. Aclaimant reserves the right to periodically modify the Product Addendum, and such modification will automatically become effective in the subsequent Renewal Service Term. 

I. Products

A. Incident and Claims Management Module - Core Aclaimant incident and claim management workflow product - includes.

  • Setup of 1 Incident Report Form and Associated Workflow 
  • Up to 3 follow-up forms
  • 1 Aclaimant Managed Dashboard

Definitions and Add-Ons:

  • Incident Report Form and Associated Workflow
    • Includes the setup of a new incident report type (i.e. injury, property damage, etc.), input form, output summary, incident report pdf document, and corresponding follow-up tasks including 2 additional discovery sessions per workflow.
  • Follow-up Form
    • Includes the setup of a workflow follow-up form (witness statement, root cause, etc.) including additional input mapping and output form creation. This is for unique follow-up forms. Follow-up forms shared across workflow will count as only 1 form.
  • OSHA Module
    • Includes access to Aclaimant OSHA record-ability module, including ability to generate OSHA 301, 300, 300a, sharps and privacy logs, API submission to OSHA, and access to Aclaimant OSHA templated dashboard. Includes (1) historical OSHA event load. Includes up to 2x/year hours and location upload.
  • Claim Submission
    • Includes (1) setup of one (1) carrier submission, including one carrier submission form if needed. Multiple carrier submissions require multiple add-ons. Includes both email and SFTP submission. API submission requires additional scoping and is subject to carrier availability and additional pricing
  • 50 State FROI Add-On
    • Includes setup of all 50 state FROIs electronically produced in the Aclaimant system.

B. Claims Analytics and Feeds Module -Core Aclaimant claim visualization and data connectivity product - includes.

  • 1 Aclaimant Managed Dashboard
  • Uploads 1 Monthly or Quarterly Carrier or TPA Loss Run Load
  • Includes 1 Line of Coverage
  • 1 Historical Data Load

Definitions and Add-Ons:

  • Aclaimant Insights
    • Enables Aclaimant's Predictive Modeling solution that includes predicted claim cost and duration, litigation likelihood and OSHA recordability.setup.
  • Transactions - Add-On
    • Enables transaction tracking in Aclaimant. Adding feeds required additionally.
  • Lines of Coverage
    • Includes an additional line of coverage on claim feeds. This is independent of the number of carrier feeds. (e.g. Work Comp, GL, Property, Auto, Builders Risk, etc.)
  • Carrier or TPA Feed Loss Run Feed (Monthly)
    • Includes additional monthly carrier loss run feed ingested by Aclaimant. Each carrier feed includes ingestion by Aclaimant via API, SFTP or other approved secure file transfer mechanism and load into the Aclaimant system for loss runs only. File must be provided in electronic format (CSV, XLSX, TXT, or Flat file) on a monthly basis. Aclaimant does not accept pdf loss runs but can recommend vendor partners who can translate these into Aclaimant useable format. Some feeds may need to be provided by customer to Aclaimant via SFTP, others may be sent directly from carrier (carrier dependent).
  • Carrier or TPA Feed Loss Run Feed (Quarterly)
    • Includes additional quarterly carrier loss run feed ingested by Aclaimant. Each carrier feed includes ingestion by Aclaimant via SFTP, data QA, claim matching and load into the Aclaimant system for loss runs only. File must be provided in electronic format (CSV, XLSX, TXT, or Flat file) on a quarterly basis. Some feeds may need to be provided by customer to Aclaimant SFTP, others may be sent directly from carrier.
  • Carrier or TPA Feed Transaction Feed (Monthly)
    • Includes additional monthly carrier transaction feed ingested by Aclaimant. Each carrier feed includes ingestion by Aclaimant via SFTP, data QA, claim matching and load into the Aclaimant system for loss runs only. File must be provided in electronic format (CSV, XLSX, TXT, or Flat file) on a monthly basis. Some feeds may need to be provided by customer to Aclaimant SFTP, others may be sent directly from carrier.
  • Carrier or TPA Feed Transaction Feed (Quarterly)
    • Includes additional quarterly carrier transaction feed ingested by Aclaimant. Each carrier feed includes ingestion by Aclaimant via SFTP, data QA, claim matching and load into the Aclaimant system for loss runs only. File must be provided in electronic format (CSV, XLSX, TXT, or Flat file) on a monthly basis. Some feeds may need to be provided by customer to Aclaimant SFTP, others may be sent directly from carrier.
  • Financial Feed (Monthly)
    • Includes monthly load of Aclaimant CSV hours template and associated fields. Must be provided through secure file share link using an Aclaimant approved template.

C. Safety and Loss Control Module - Core Aclaimant safety management workflow product - includes.

  • Up to 3 Safety Forms and Associated Workflows
  • Up to 1 Safety Follow-up form
  • 1 Aclaimant Managed Dashboard

Definitions and Add-Ons:

  • Safety Report and Associated Workflow
    • Includes the setup of a new safety report workflow (observation, audit, inspection form) and corresponding follow-up scoring and tasks including 2 additional discovery sessions per workflow.
  • Safety Follow-up Form
    • Includes the setup of a workflow follow-up form (statement, root cause, corrective action) including additional input mapping and output form creation. This is for unique follow-up forms. Follow-up forms shared across workflow will count as only 1 form.

D. Aclaimant Dashboards and Analytics- Aclaimant core data visualizations - definitions and add-ons

  • Templated Analytics Dashboard 
    • Includes prebuilt dashboards without changes
  • Aclaimant Managed Analytics Dashboard
    • Includes one (1) initial setup of customer configured dashboard including up to 15 widgets (chart, graph, table). Ongoing adjustments are included in monthly hourly allocation. Only Aclaimant managed dashboards may be configured or adjusted. Includes up to 4 meetings with our analytics and dashboard team.

III. Technical Add-Ons

  • SSO 
    • Includes setup of login based SSO, either OAuth2.0 or SAML. Allows user login via SSO only, does not include user provisioning. Bulk user uploads fall into monthly success allocations - see Aclaimant Services for more information.
  • Data Warehouse
    • Includes setup of Aclaimant data warehouse. Data updated 4x/day and customers able to pull data via ETLs through ODBC or JDBC up to 4x/day, as written by customer or third party. Data warehouse includes access to the following standard objects: incidents, claims, loss runs, policies, policy periods, lines of coverage, events, safety reports, tasks and companies. Today, Aclaimant data warehouse does not support access to artifacts (files created or stored in the Aclaimant system), including attachments, and notes entered within the Aclaimant system.

IV. Integration Add-Ons

  • Base - Integration Access
    • Use of any core module (above), provides access to integrations. Each integration needs to be added as a separate item.
  • Integration - Existing
    • Includes addition of (1) existing integration type (HRIS, Intake, Location, or Asset) with (1) existing integration vendor partner available in Aclaimant's partner marketplace (ProCore, BambooHR, etc.).  Please see Aclaimant Partners Page for more information.
  • Integration - New Vendor To Existing Type Of Integration
    • Includes addition of (1) existing integration type (HRIS, Intake, Location, or Asset) with (1) new integration vendor partner. Please note, additional development time will incur hourly fees below for the setup of any new vendor integration.
  • Integration - Flat File Integration
    • Includes addition of (1) existing integration type (HRIS, Intake, Location, or Asset) with (1) new integration vendor partner utilizing our Flat File Integration integration type. This integration type allows lookup on intake of any flat file provided via SFTP on any frequency.
  • Integration - Net New Type of Integration
    • Includes addition of (1) new integration type (not HRIS, Intake, Location, or Asset) with (1) any vendor partner. This integration type must be approved by product and will incur one off scoped costs.Integration
  • Integration Team Development Time For Adjustments To Existing Integrations OR Creation of New Vendor Integration
    • Adjustment requests to any existing integration or existing integration type with new vendor that requires development work will be billed hourly.
